1. Select an appointment option from the list, then click or tap Schedule.
2. Choose a date and time, then click or tap Next.
3. Enter your contact information, then click or tap Schedule Event.

Refresh the page to start over.

Hours Monday-Friday 9:00-5:00 MST. Contact us for Saturday appointments.

Please call to cancel or reschedule, or use the Reschedule and Cancel buttons in your appointment confirmation email.
